Call any of our three location – see Contact Us - and make an appointment to attend one of our information sessions. At the information session you will learn more about the Transitions Program, fill out an application, and have a brief private meeting with one of our staff to discuss whether the program is suitable for you. You are required to pre-register for the information session so please call ahead.

You'll need a referral...

Prior to or after attending our information session you will need to book an appointment to meet with a Case Manager/Employment Counsellor at an Employment Resource Centre (ERC). In order to confirm your application for our program, we require a copy of an ‘Action Plan/Referral Form’ from them indicating ‘Career Planning/Work Exploration’ as your next step. As there are many services available in the community, Case Managers are there to help you determine/verify what type of service will best meet your needs and follow up with you as you attend one or more programs. If you've already been to our information session, you can let them know that you're interested in our program and have applied.
